Something is very wrong with one of your fish. It’s hanging at the surface gasping, or lying on its side at the bottom, or its colour has faded and it’s not swimming. You need to act now — the difference between saving and losing a fish is often the first 30 minutes of intervention.

1. Multiple fish gasping at the surface — chemistry emergency
Cause: Ammonia or nitrite spike, or dissolved-oxygen crash. Multiple fish affected = system problem, not individual disease.
Immediate action (do all of these):
- Test water NOW with API Freshwater Master Test Kit. Ammonia, nitrite, nitrate, pH.
- 50% water change immediately with dechlorinated water at matching temperature. Use Seachem Prime at 5× dose — it detoxifies ammonia and nitrite temporarily while you sort the underlying issue.
- Increase aeration — add an air stone with air pump if you don’t already have one, or angle your filter output upward to break surface tension.
- Check for dead fish behind decor — a decomposing body causes ammonia spikes. Remove immediately.
- Check filter — power out, impeller jammed, or media replaced recently killing beneficial bacteria?
- Stop feeding for 48 hours.
- Do a second 25% water change 12 hours later.
- Test daily until all levels stabilise.

2. Single fish clamped, lethargic — disease onset
Cause: Bacterial infection, ich, velvet, or internal parasites. Usually one fish first, then spreads.
Diagnose further:
- White spots on body and fins? → Ich. See our ich treatment guide.
- Fine gold dust in torchlight? → Velvet. Treat with copper-free medication.
- Ragged fin edges, fins eroding? → Fin rot. See our fin rot guide.
- White fluff on body? → Fungus. Treat with NT Labs Anti-Fungus.
- Bulging scales (pinecone)? → Dropsy. Prognosis poor.
- Rapid gill movement, no marks? → Gill parasites or poor water quality.
Immediate action:
- Test water — poor water quality suppresses immunity and makes disease worse.
- 25% water change if water quality is fine, 50% if not.
- Diagnose specifically (see disease overview: common fish diseases).
- Treat with appropriate medication — usually Waterlife Myxazin, NT Labs Anti-Fin Rot & Fungus, or eSHa 2000.
- Remove activated carbon from filter before dosing (carbon strips medications).
- Follow full course even if fish appears better after 2 days.
3. Fish sideways / floating / sinking — swim bladder
Cause: Constipation, bacterial infection, physical trauma, or genetic deformity in certain fancy strains.
Immediate action:
- Fast the fish for 2–3 days. Don’t feed.
- On day 3, feed a blanched pea — skin removed, mashed. Just a piece the size of the fish’s eye. Helps constipation.
- Increase temperature slightly (26–27°C) — helps digestion.
- Reduce water flow temporarily — buoyancy-struggling fish get exhausted fighting current.
- If no improvement after 5 days: treat with Waterlife Myxazin for bacterial swim bladder infection.
Common in: fancy goldfish (bred with compressed body shape), bettas that overfed, balloon mollies.
4. New fish gulping air at surface — chemistry mismatch
Cause: Newly-added fish going into shock from parameter differences between shop water and your tank.
Immediate action:
- Do NOT do a big water change. That further shocks the fish.
- Verify tank parameters with API test kit. Match to shop conditions if possible.
- Turn tank lights off — stress reduction.
- Add air stone — extra oxygen, less exertion required.
- Add Seachem Prime at 2× dose — detoxifies any residual ammonia and reduces stress.
- Wait 24–48 hours. Fish either acclimate or die at this stage; further intervention rarely helps.

5. Fish on bottom, breathing slowly — severe illness or old age
Cause: End-stage illness (advanced infection, organ failure, cancer), severe injury, or genuine old age.
Assess:
- Is the fish old? (Guppy 2+ years, tetra 3+ years, angelfish 5+ years, goldfish 8+ years, bristlenose 8+ years).
- Was there a triggering event? (Predator, filter injury, aggression).
- Any obvious symptoms? (Ulcers, tumours, extreme thinness).
If old and terminal: hospice care. Move to a peaceful low-flow area of the tank. Feed if it eats. Consider euthanasia if suffering.
If sudden and recent trigger: as for section 2 — treat for likely infection.
Humane euthanasia when appropriate:
- Clove oil overdose — clove oil (eugenol) from UK pharmacies. Dose 400 mg/L (about 10 drops in 1 litre) to anaesthetise; wait 15 minutes; add same amount again to overdose. Fish passes peacefully. This is the standard UK humane method.
- Do NOT flush live fish — cruel and illegal under UK animal welfare legislation.
- Do NOT put in the freezer — slow suffocation, painful.
- Do NOT hit with a hammer unless you’re 100% confident. Traditional but stressful if botched.
